It worked!

I had plenty of information about the importance of the pitch and the general idea of what to include, but when it came to the actual wording THIS resource was gold. I just filled in the blanks (mad lib style), chose my photos, and sent along to a publisher. I heard back almost immediately that my pitch was perfect and that she would keep me in mind for future issues. (update: I heard from her last night that I'm on dock for their fall issue!). Thanks HYDC!
